What if I need financing for a procedure?
If you use third-party financing for treatment, the membership discount is reduced by 5%. Payment options for membership dues are also available upon request.

- Does not apply with dental specialists outside HAD
- Injuries covered by workers’ comp, disability, lawsuit, or outside medical care are not covered
- Plan is non-transferable and nonrefundable

- Payments due at time of appointment reservation
